The Personal Identity Verification (PIV) Enrollment Official serves as a Sponsor proxy by performing identity proofing for Applicants located remotely from the Sponsor or Registrar.

 

TRAINING:           Click here to access the training required before a PIV Enrollment Official can perform identity proofing and process a PIV request for an Applicant.  Be sure to print out and retain your certificate of completion with your Enrollment Official ID Number.  You will need this number to perform your role as PIV Enrollment Official.

Required training should be completed before the PIV process takes effect on

For new PIV card issuance, you must:

 

 

STEP 1:                Receive a signed PIV Request Form from the Sponsor.

 

STEP 2:                Receive in person from the Applicant:
 

·     Completed background investigation documents (SF 85, SF 85P, or SF 86) and Form I-9, Employment Eligibility Verification with Section 1 completed by the Applicant.

·     Two completed fingerprints cards (SF 87 for employees; FD 258 for all others).  If Applicant arrives without completed fingerprint cards, provide them with the appropriate blank cards and send them to a local law enforcement facility for fingerprinting.*

 

*Fingerprint cards may be obtained from the PIV Sponsor or if needed, by contacting the Office of Security (see above for contact information). 

 

·     One passport-quality photo or a digital image taken by the Enrollment Official; facial image should meet the requirements specified in SP 800-76.

·     Applicant’s PIV training certificate.

 

STEP 3:                Performing identity proofing:
 

·     Review background investigation documents for completeness and signatures.

·     Visually inspect, validate, and copy identity source documents; verify that picture on identity documents is of Applicant.  Applicants will complete and sign the top portion (Section 1) of the Form I-9.  After the Applicant completes the top portion of the form, you will carefully inspect the Applicant’s two forms of ID (one must be a Federal or State-issued photo ID).

·     Complete and sign Section 2 of the I-9 Form.

·     Complete and sign Section B of the PIV Request form.

 

      STEP 4:                Send to the Sponsor by secure delivery means (e.g. Registered Mail/FedEx):

 

·     PIV Request Form

·     Completed Form I-9, with copy of two ID source documents

·     Completed background investigation documents (SF 85, SF 85P, or SF 86)

·     Completed fingerprint cards

·     Photo

·     Applicant’s PIV training certificate

     

 

      For Card Renewals:  

 

      STEP 1:                Receive signed PIV Request Form from the Sponsor, with verification of favorable background investigation.

      STEP 2:                Ensure that Applicant has taken the PIV Applicant training module by viewing the certificate of completion.  Training is available on the PIV Applicant page,                                                            www.osec.doc.gov/osy/hspd12/applicants.htm.

 

      STEP 3:                Receive or take an updated photo of the Applicant that meets the requirements specified in SP 800-76 

 

      STEP 4:                Identity proof upon Sponsor’s request if no Form I-9, Employment Eligibility Verification is on file:

 

·     Review background investigation documents for completeness and signatures, if necessary.

·     Updated photo (you may also e-mail an electronic photo to the Sponsor)
