The Personal Identity Verification (PIV) Remote
Issuer is not located at or near a PIV Card Issuing Facility (PCIF)
and serves as a proxy to the PCIF
Issuer by delivering personalized PIV cards to authorized Applicants who are also remote to the
PCIF.
TRAINING:

PIV card to an Applicant. Be sure to print out and retain your certificate of completion with your
Remote Issuer ID Number. You will need this number to perform your
role as PIV Remote Issuer.

Each time you deliver
a PIV card to an Applicant, you must:
STEP 1: Receive the newly personalized PIV card and the PIV
Request Form from the PCIF Issuer.
STEP 2: Notify the Applicant that the PIV Card is ready for
pick-up.
STEP 3: Verify that the individual appearing in person to
receive the PIV card is the Applicant by:
· Comparing the photo on the State or
Federal-issued ID with the Applicant, and
· Ensuring that both the photo and
name match those on the PIV card.
STEP 4: Fill out Section D (Card Details) portion of the PIV
Request Form (including the shaded area), sign the Issuer Signature block, and
date.
STEP 5: Ensure the Applicant has received notification of
Applicant rights and responsibilities from the PIV Sponsor.
STEP 6: Have Applicant sign Section E (Applicant
Acknowledgement) of the PIV Request Form, thereby accepting the PIV card and
related responsibilities.
STEP 7: Return the original PIV Request Form to the
Registrar.
