The Personal Identity Verification (PIV) Sponsor acts on behalf of the Department or agency to request an Applicant be issued a PIV card after completion of appropriate identity authentication and background checks.

 

TRAINING:           Click here to access the training required before a PIV Sponsor can request a PIV card for an Applicant.  Be sure to print out and retain your certificate of completion with your Sponsor ID Number.  You will need this number to perform your role as PIV Sponsor.

For PIV Applicants who are located at or near the Sponsor:

 

Each time you request a PIV card for an Applicant, you must:

 

STEP 1:                Click here for the PIV Request Form (free Adobe Acrobat reader).  Complete and sign Section A of this form.  SF 85 Questionnaire

 

STEP 2:                Direct the Applicant to the PIV Applicant page, www.osec.doc.gov/osy/hspd12/applicants.htm, which provides links to training and background investigation paperwork the Applicant must complete. 

 

·     Note that training is currently available in online format only.  Please contact the Office of Security (see contact information above) if you cannot access the training. 

·     Paperwork can also be provided to Applicants in person and through the mail, as well as via e-mail.

 

STEP 3:                Look at the Applicant’s position sensitivity to determine which background investigation form (SF 85, SF85P, or SF 86) the Applicant should use.  Choose one of the following forms:

STEP 4                 Provide to Applicant before their arrival:

 

·     The appropriate background investigation form (SF 85/SF 85P/SF 86)

 

·     The OF 306, Declaration for Federal Employment form (click here for the form)

·     Two SF 87 Fingerprint Cards:  For current and prospective DOC employees*

·     Two FD 258 Fingerprint Cards:  For current and prospective DOC contractors and visitors*

 

* Check with the Registrar to determine how the Applicant should provide fingerprints.  If fingerprinting capability does not exist at the PIV Card Issuing Facility (PCIF), you will need to provide fingerprint cards.  Fingerprint cards may be obtained by contacting the Office of Security, if needed (see contact information above).  These cards are not available electronically.

 

·     I-9 Form, Employment Eligibility Verification

·     Instructions to report with two identity source documents (one of which must be a Federal or State-issued photo ID)

  

STEP 5:                Perform identity proofing:
 

·     Ensure Applicant completes and signs the top portion (Section 1) of the I-9 Form, Employment Eligibility Verification.

·     After the Applicant completes the top portion of the form, visually inspect, validate, and copy Applicant’s two forms of ID (one of which must be a Federal or State-issued photo ID).

·     Verify that the photo on the identity documents is that of the Applicant.

·     Complete and sign Section 2 of the I-9.

 

STEP 6:                Finalize documentation:
 

·     Check to make sure the Applicant has completed all required paperwork correctly.

·     Obtain a copy of the Applicant’s PIV training certificate of completion.

·     Complete and sign Section B of the PIV Request Form.

 

STEP 7:                Provide Applicant with Privacy Act information, appeals process, and rights and responsibilities.

 

STEP 8:                If the Applicant is at or near their servicing PCIF, have them report to the Registrar for fingerprinting and photo.  If Applicant is not near the PCIF, direct them to obtain fingerprints from a local law enforcement facility, to obtain a passport-quality photo, and to bring both to you.

 

STEP 9:                Deliver to the Registrar, in person or by secure means (e.g., Registered Mail/Federal Express), the packet containing the background investigation forms, PIV Request Form, photocopy of two IDs, I-9 form, training certificate, fingerprint cards (if applicable), and passport-quality photo (if applicable).

 

STEP 10:              Inform the Applicant of the background investigation results, once completed.

  

 

 

For PIV Applicants who are NOT located near the Sponsor:

 

Each time you request a PIV card for an Applicant, you must:

STEP 5:                Instruct the Applicant to report to a designated Enrollment Official with the following:
 

·     I-9 Form, with the top portion (Section I) completed and signed

·     Two IDs, one of which must be a Federal or State-issued photo ID

·     Completed SF 85/SF 85P/SF 86 (whichever is the appropriate background investigation form)

·     OF 306, Declaration for Federal Employment

·     Fair Credit Reporting Act form

·     Completed fingerprint cards

·     Passport-quality photo (unless Enrollment Official has the capability to take a digital quality photo), which meets the requirements specified in SP 800-76

·     Applicant’s PIV training certificate

 

STEP 6:                Send the PIV Request Form to the Enrollment Official.

 

STEP 7:                Receive from the Enrollment Official:

 

·     PIV Request Form, signed by the Enrollment Official

·     Completed I-9 Form, with photo copy of two ID source documents

·     Completed background investigation documents

·     Completed Fingerprint cards

·     Photo

·     Applicant’s PIV training certificate

 

STEP 8:                Check to make sure the Applicant has correctly completed and signed all required paperwork.  Ensure the Enrollment Official has completed Section B of the PIV Request Form.

 

STEP 9:                Deliver to the Registrar by secure means (e.g., Registered Mail/Federal Express) the packet containing the PIV Request Form, I-9 form, photocopy of two IDs, background investigation forms, fingerprint cards, passport-quality photo, and training certificate.

 

STEP 10:              Inform the Applicant of the background investigation results, once completed.

 

 

For Card Renewals:

 

STEP 1:                Check to see if the Applicant has an I-9 Form on file, as well as a completed background investigation.

 

STEP 2:                Complete the following:

·         If the Applicant has an I-9 Form and a completed background investigation on file, submit a copy of the I-9 and an original signed PIV Request Form (with investigation information filled in) to Enrollment Official and/or Registrar.

 

·         If the Applicant does not have a completed background investigation or I-9 on file, then complete the ten (10) steps listed above.

 

·          If the Applicant has a completed background investigation on file, but no I-9 on file, then complete the identity proofing steps as listed above or instruct the Applicant to report to the Enrollment Official for identity proofing.

 

·         If the Applicant has on file an I-9 form but no completed background investigation on file, then complete the background investigation steps listed above.

 

STEP 3:                Complete steps 6 through 10 above as applicable.
