arrowTraveling Abroad - Passports


Official passports and visas are required for travel to almost every country. The OAS Travel Management Division includes the passport officials who will assist you with this process.

The State Department’s Passport Office requires four weeks to process an official or diplomatic passport application. If you need to have the passport application expedited,
a letter is required explaining in detail the circumstances for the expeditious handling.

The official or diplomatic passport is Government Property and not personal property. The passport is to be used for official business only. A tourist passport must be used for personal travel. When you are not performing official travel, the passport will be kept on file in the ADTRAV Travel Office, Room 2069.

Required Documents for New or Renewed Official and Diplomatic Passports

Employees must complete one of the following documents:

  • Passport application (DS-11). This passport application is for employees who do not have a tourist passport. Complete this document, but do not sign it. HCHB employees must visit the OAS Travel Management Division’s Passport Officer.

    - OR -

  • Passport application (DS-82). Complete and sign. This form can only be used if your most recent passport was issued less than 15 years ago and you were at least 16 years of age when the passport was issued. If so, include the old passport with your application. If your name has changed since that most recent passport had been issued, complete a DS-19 and attach it, along with official documentation supporting the name change (e.g., marriage license, court order, etc.) to the application.

If you have never been issued a passport, you will fill out the DS-11application. If your passport is about to expire, fill out the DS-82 application.

Be sure to follow the instructions on the State Department’s website for printing and completing the appropriate form.

In addition, employees must provide all of the following documentation:

  • Two passport photos, not over six months old. Photos must be identical and exactly measure 2”x 2”). For HCHB employees, the Office of Security in room 1033 will take photos for you. To request this service, you must apply in person and present one of the following documents: travel authorization, CD-97, or Supervisor’s authorization letter.

    - AND –

  • CD-97, Request for Security Assurance and Official Passport Clearance for Foreign Travel. This form must be typed and all applicable questions must be answered. The form must be cleared through your bureau’s security office.

    - AND –

  • Proof of citizenship. The passport application package must include a valid or expired US passport OR a certified copy of your birth certificate (this means a raised seal must be on the document) OR your original naturalization certificate.

    - AND –

  • Copy of the most recent Personnel Action. The traveler’s most recent Personnel Action, and an Oath of Office is required for consultants, experts, etc. Contractors are not authorized to have official passports issued to them, since they are not Government employees.

Amending an Official or Diplomatic Passport

An official or diplomatic passport can be amended if the employee’s name has changed, additional visa pages are required, or a limited passport must be extended. Complete the
 CD-97. Note: If the limited passport belongs to a consultant or expert, a new personnel action and oath of office is also required.
